CoagCare® by ZyCare®, Inc. is the first and only FDA 510(k) Cleared anticoagulation program for oral warfarin/Coumadin® management for clinic and Patient Self-Testing management. CoagCare weekly Patient Self Testing has been demonstrated in FDA clinical trails to provide an increase in Time in Therapeutic Range (TTR) which is an important factor in minimizing strokes and bleeding complications of warfarin therapy. CoagCare is provided to the medical community as a web-based, application service provider (ASP) service. Data is maintained on CoagCare’s secure servers which are encrypted and ‘backed-up’ each day to ensure security of information.
ZyCare’s Corporate Privacy Policy does not allow the sale or sharing (outside of ZyCare or your caregiver or insurer) of Identifiable (individual patient or caregiver) information under any circumstances except when required by law. ZyCare adheres to strict HIPAA ( Health Insurance Portability and Accountability Act of 1996, Public Law 104-191 (“HIPAA”)) compliance.
ZyCare will never provide information to third parties that identify a patient, care giver or clinic except when required by law. ZyCare may provide or use bulk data for scientific research, clinical review, customer support performance or marketing but this data will have all patient caregiver and clinic information ‘blinded’ in accordance with HIPAA requirements.
Privacy
Employees and Management of ZyCare, Inc. understand, recognize and respect the importance of customer privacy. All staff must sign a privacy statement as part of their employment agreement. Management and staff understand that failure to strictly adhere to the requirements of this statement are grounds for dismissal.
Physician/Caregiver/Institution Policy

ZyCare has a Business Associate Agreement in place with each customer (ASP Lessee) that is using CoagCare as required by the HIPAA Act and Regulations.  If required by governing law or the specific medical institution, the customer is responsible for obtaining patient consent to store Identifiable Patient Information on CoagCare.

Security
Zycare, Inc. has developed its’ CoagCare system to meet or exceed standards of customer security and current laws in the areas of privacy of customer and patient data.
 
This is achieved by:
A state of the art server security firewall.
  Installation and maintenance of CoagCare servers and offsite equipment in a physically secure, technologically advanced computer facility.
  Password and User restricted access site entry via a 128 bit encryption portal.
Internal authentication processes to enter the web site via unique user ID and personal password
